From time to time, Perennial Recovery may be required by law to provide you with specific written information (collectively, “Required Disclosures”). By agreeing to these terms, you are consenting to Perennial Recovery’s provision of such Required Disclosures to you electronically through the Perennial Recovery Patient Portal, in lieu of receiving paper hard copies.
If you consent to receiving Required Disclosures from us electronically, your consent applies to any future Required Disclosures regarding your patient account. At our discretion, we may also mail any Required Disclosures in paper form to your mailing address on file if we choose to do so.
Getting Paper Copies
You may request a paper copy of anyrecord provided or made available electronically to you by us. You may contactus to request a copy at 888-960-6225. Paper copies will be provided,though there may be a nominal fee involved. Please inquire about any fees whenrequesting paper copies.
Withdrawing Your Consent
If you prefer to receive only paper hard copies of any required disclosures, you may withdraw your consent via the Perennial Recovery Patient Portal or by contacting us at 888-960-6225. Supposeyou elect to receive the Required Disclosures in paper format only. In thatcase, it may slow the speed at which we can deliver services to you, as we willneed first to send the required notices or disclosures to you in paper formatand then wait until we receive acknowledgment of your receipt of such papernotices or disclosures.
Hardware and Software Requirements
To access and retain required disclosures electronically, you will need Internet access, a valid emailaddress, and a printer or sufficient computer disk space to download a copy ofany Required Disclosures we may provide. In addition, you must have a computeror mobile device that supports one of the following:
